Camper Report of Physical Exam & Immunizations:
A copy of a physical exam report completed and signed by the physician, including the date of the last physical exam and the list of immunizations must be submitted before attending Camp. The physical exam date must be within 18 months prior to when your child is expected to attend Camp. If the last physical exam is not within that time frame, please make an appointment far enough in advance to ensure that the required forms will be received by the deadline. The immunizations required by TFA/TA Camp are the same as those required for attendance at school.

Authorization To Administer Medication to a Camper:
For campers who will require medications at Camp, the Medication Authorization Form must be completed and signed by the parent/guardian for each medication. The parent/guardian should deliver the medication(s) directly to the Camp Director on the first day of the child’s scheduled camp session(s). Medications must be in the original prescription container with the prescription label affixed, noting the exact dose of the medication to be dispensed.
